About Connecticut Community Care Inc
Connecticut Community Care, Inc., (CCCI), identifies choices and provides services to help people of all ages, abilities and incomes to live at home. Founded in 1974 and incorporated in 1980, CCCI is a private, nonprofit providing care management services to 12,000 clients and their families each year. Professional care managers conduct comprehensive assessments, develop, monitor and adjust care plans and arrange in-home services so that clients can live safely and independently at home in a cost-effective manner as an alternative to costly nursing home care.
Based in Bristol, CT, CCCI has three regional offices (Watertown, Wethersfield and Franklin, CT) and is the access agency to the state- funded CT Home Care Programs for Elders/Disabled (CHCPE/CHCPD) in 124 towns in the state's Northwest, North Central and Eastern regions. CCCI has helped more than 250,000 clients and families and has saved the state and its taxpayers millions of dollars by averting premature nursing home placement.
